INSTRUCTIONS FOR COMPLETING
1. Download and complete the attached form --- be sure to sign and date application in BOTH places.
2. Send completed application with non-refundable $25.00 check/money order made payable to
“The 500 Club Apartments”

3. Upon receipt of your application, and based on availability, you will be assigned to a 500 Club unit. The status
of your application will be sent to you via email.
4. If your application is accepted, you will receive a lease agreement. You will need to return the lease agreement
by the date specified with a check /money order for the security deposit which is equivalent to one month’s
lease payment. It the lease agreement is not returned by the date specified, your unit will be released and made
available to the next applicant. A new application will then need to be submitted, if you wish to reapply.
